Housing Resources
  • East Toledo Family Center: (419) 691-2254
    Ensures that Toledo area seniors have a safe home (windows, gutters, plumbing, furnace, railings, tubs, etc.
     
  • Great Lakes Community Action Partnership: (419) 333-6049
    Offers help with home repairs, energy upgrades, water well safety and improvement, as well as home energy conservation assistance (insulation, energy-efficent refrigerator/freezer, light bulbs, and water measures). Also offers veteran housing support. 
    Does not serve Lucas County.
     
  • Lucas County Land Bank: (419) 213-4293
    The Home Preservation Initative helps homeowners maintain and repair their home (home visits, free general advice, contractor guidance, and access to low-interest loans).
     
  • The Ability Center of Greater Toledo: (419) 885-5733
    Provides home modification projects that make homes of people living with disabilities more accessible (temporary ramp, permanent ramp, low rise steps, stairlifts, grab bars). 
     
  • Area Office on Aging: (800) 472-7277
    Provides quality services that allow you and your loved ones to be as independent as possible; empowering you to live your life to the fullest. 

    Housing Search: Helps identify the best housing fit for you by estimated cost, location, and move-in timeframe.

    Home Energy Assistance Program: Provides a one-time benefit to eligible Ohioans with cooling assistance during the summer months.

    Home Repair: A state-funded program for minor home repairs and modifications for eligible older adults (60+). 
     
  • Fair Housing Center: (419) 243-6163
    Provides services that protect renters and homebuyers (protections for persons with disabilities, service and assistance animals, homebuyer protections, renters rights, landlord-tenant mediation, reasonable accomodations and modifications).
     
  • Lucas County Homelessness Board: (419) 244-9440
    Provides programs and resources to accelerate solutions to end homelessness (housing integration, case managers, housing solutions and opportunities, housing assistance).

Food Assistance


Personal and Family Care

  • Adult Protective Services (Lucas County Job and Family Services): 1-844-640-6446
    If you suspect an adult is experiencing abuse, neglect, or exploitation, please make a report by calling 855-644-6277 (reports are anonymous. 

    Adult Protective Services investigates and evaluates all reports of suspected abuse, neglect, and exploitation of adults age 60 and older. When funding and services are available, adults who experience abuse, neglect and/or exploration are offered support for protection and self-sufficiency. 
     
  • Publicly Funded Child Care (Lucas County Job and Family Services): 1-844-640-6446
    Provides financial assistance to eligible parents and guardians to help with child care costs while they engage in work, education, or receiving job training. 
     
  • Area Office on Aging: (800) 472-7277
    Provides quality services that allow you and your loved ones to be as independent as possible; empowering you to live your life to the fullest. 

    Transportation Search: Area Office on Aging provides transportation search options to help look for the cheapest, quickest, or best way to get transported across town.

    Family Caregiver Support Program: This program includes information to caregivers about available services, assistance to caregivers in gaining access to services, counseling, support groups, caregiver training, respite care to enable caregivers to be temporarily relieved from their caregiving responsibilities and more.

    In-Home Care Program: Allows low-income, older adults to live in their own homes by providing home-delivered meals and a home health aide to help with bathing and dressing. Other services include minor home modification, emergency response systems, home medical equipment and supplies, adult day services, tranportation and more.

    Kinship Navigator Program: Supports kinship and grandfamilies in Lucas County who raise children when the biological parents are unable or unwilling to do so and is nationally recognized.

Medical Resources

  • Medical Assistance (Lucas County Job and Family Services): 1-844-640-6446
    Medicaid is a state and federally funded health program for certain low-income and medically vulnerable people. As an entitlement program, those who meet all income and eligibility requirements are guarenteed coverage through the Medicaid program.
     
  • HealthChek (Lucas County Job and Family Services): (419) 213-8000
    Ohio's Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program designed to discover and treat health problems early. It provides a group of services to children and teens (birth through age 20) which include: prevention, diagnosis and treatment. 
     
  • Pregnancy Related Services (Lucas County Job and Family Services): 1-844-640-6446
    Open to all pregnant women receiving Medicaid. Pregnancy Related Services (PRS) workers remind, refer, encourage, and arrange prenatal care. Transportation is also avialable if necessary. Some women are provided follow-up services for one year after their child's birth.
     
  • Non-Emergency Medical Transportation (Lucas County Job and Family Services): (419) 213-8910
    Lucas County Job and Family Services contracts with local cab companies to provide eligible Medicaid recipients free transportation to and from medical appointments to Ohio Medicaid providers located in Lucas County. Call (419) 213-8910 for more information on how to apply. 
     
  • Long Term Care Services (Lucas County Job and Family Service): 1-844-640-6446
     Nursing Home and Institutional Care: The best solution for someone with a severe disability or a fragile medical state may be a nursing facility. They provide care for individuals who cannot care for themselves at home and assist with daily living activities. 

    Home and Community-Based Waiver Services: The program help Ohio Medicaid consumers remain at home isntead of being in a nursing home, hospital, or facility for people with developmental disabilities.
     
  • Prevention Retention & Contingency (PRC) Program: 1-844-640-6446
    The PRC Program is designed to provide benefits and services to address a discrete crisis or episode of need, lessening the risk that the issue will create the need for additional supports/long-term benefits if it goes unaddressed. PRC support is short-term and is not intended to meet ongoing needs. 

    Support may include employment materials (uniforms, footwear, tools, equipment, professional licensure, testing fees, etc.), GED incentive, vehicle repairs, utilitiy assistance, disaster assistance, and public health emergency assitance.
    Each service has its own specific set of rules and guidelines as noted above.
     
  • Benefit Bridge (Lucas County Job and Family Services): (419) 213-6314
    The Lucas County Benefit Bridge program is designed to help individuals and families, who experience a significant reduction or loss of SNAP benefits due to wage increases, continue their path to self-sufficiency.

Veteran Support

  • Great Lakes Community Action Partnership: (419) 333-6049
    Provides veteran housing support for veterans Offers help with home repairs, energy upgrades, water well safety and improvement, as well as home energy conservation assistance (insulation, energy-efficent refrigerator/freezer, light bulbs, and water measures).  
    Does not serve Lucas County.
     
  • Lucas County Veterans Service Commission: (419) 213-6090
    Assists veterans and their families in times of need (food vouchers, financial relief/assistance, medical perscription funds, transportation, home repairs/roofing, etc.)
     
  • SeaGate Food Bank of Northwest Ohio: (419) 244-6996
    Distributes food to pantries and provides food assistance programs for seniors, veterans, families, and families in recovery. Veterans can receive food parcels from SeaGate's on-site pantry of choice on a monthly basis or as needed.
